Key Responsibilities:
- Provide care and services in accordance with individualized plans of care
- Teach patients and families exercises and self-care techniques
- Coordinate and collaborate with other healthcare team members
- Document care accurately and in a timely manner
- Maintain patient confidentiality and follow HIPAA regulations
- Attend continuing education and departmental training
- Promote a positive image of NWVNA through excellent patient care
Qualifications:
- Current PT or PTA license from the Massachusetts Board of Allied Health
- Minimum 2 years experience providing direct patient care in a community or clinical setting
- Associate’s degree and graduation from an accredited Physical Therapy program
- Current valid driver’s license
- Strong communication skills and ability to work independently and as part of a team
- Comfortable using computer technology for documentation and communication
